1910.95(m)(1) "Exposure measurements." The employer shall maintain an accurate record of all employee exposure measurements required by paragraph (d) of this section. 29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program. a) USS Manchester, Flight Deck: Employees operating the pneumatic needle guns were exposed to 775.9% of the action level of 85 (8-hour time weighted average) or an equivalent sound level of 104.7 dBA during the sampling on May 8th, 2025. There was no noise monitoring conducted, which potentially exposed employees to noise hazards. b) USS Manchester, Flight Deck: Employees operating the pneumatic needle guns were exposed to 548.6% of the action level of 85 (8-hour time weighted average) or an equivalent sound level of 102.2 dBA during the sampling on May 8th, 2025. There was no noise monitoring conducted, which potentially exposed employees to noise hazards. c) USS Manchester, Flight Deck: Employees operating the pneumatic needle guns were exposed to 316.5% of the action level of 85 (8-hour time weighted average) or an equivalent sound level of 98.3 dBA during the sampling on May 8th, 2025. There was no noise monitoring conducted, which potentially exposed employees to noise hazards.

29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer. USS Manchester, Flight Deck: On or about May 8th, 2025, and at times prior to, employees conducted needle gun operations while wearing the 3M Series 6000 air purifying full face respirators with P100 cartridges without cleaning the respirators according to the manufacturer's recommendations. This condition potentially exposed employees to dermal and respiratory hazard(s).

USS Manchester, Flight Deck: On or about May 7th, 2025, and at times prior to, employees conducted needle gun operations while wearing the 3M Series 6000 air purifying full face respirators with P100 cartridges without checking for cleanliness, wear and tear, and duct tapes utilization instead of assigned inhalation and exhalation valves. This condition potentially exposed employees to respiratory hazard(s).
