29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about: May 9, 2025 Location: 4 Fox Lane Spring Valley, NY 10977  a) At least 3 workers were inside an excavation site without adequate or effective protection from cave-ins. The excavation site was approximately 10-15 feet in depth and 40-55 feet in width and length. Workers exposed to crushing hazard due to excavation collapse.
