29 CFR  1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work.   On or about May 09, 2025, during a commercial construction project located at 2022 Fairview Court, Cleveland, OH 44106, the employer did not ensure employees working at a height of approximate 11.2 feet during scaffolding dismantling were trained by a competent person.
