29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about May 09, 2025, during a construction roofing project on a commercial building located at  6850 Mayfield Road, Gates Mills, OH 44040, the employer did not ensure employee(s) were wearing protective helmets where there is a possible danger of head injury from falling and flying debris while working directly underneath an active roof replacement.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  On or about May 09, 2025, during a construction roofing project on a commercial building located at  6850 Mayfield Road, Gates Mills, OH 44040, the employer did not ensure employees working on a steep roof were protected from various fall heights of approximately 21 feet and greater than 25 feet, by utilizing a guardrail systems with toe boards, safety net systems, or personal fall arrest systems.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:   On or about May 09, 2025, during a construction roofing project on a commercial building located at  6850 Mayfield Road, Gates Mills, OH 44040, the employer did not ensure employees working on a steep roof were protected from various fall heights of approximately 21 feet and greater than 25 feet, by utilizing a guardrail systems with toe boards, safety net systems, or personal fall arrest systems. Employees were not provided training in fall protection by their employer.
