29 CFR  1926.451(g)(1):   Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:     (a) Barndominium west side; On or about May 7, 2025, one or more methods of fall protection was not in place while an employee was elevated on a platform attached to the forks of a SkyTrack 8042 telehandler, that was raised to the west side of the lower roof, at a height approximately 12 feet 6 inches above ground level.

(b) Barndominium lower roof; On or about May 7, 2025, one or more methods of fall protection was not in place while employees handled material and accessed/egressed the upper roof, on the west side of the 4:12 pitch lower roof, at a height approximately 12 feet 6 inches above ground level.
