29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about May 13, 2025, employees engaged in residential roofing activities on the job site located at 11855 South 113th Ave, Papillion, Nebraska 68046, were exposed to fall hazards in that multiple employees were observed removing shingles from a residential structure at heights greater than 10 feet 4 inches without any form of fall protection.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about May 13, 2025, employees engaged in roofing operations located at 11855 South 113th Ave, Papillion, Nebraska 68046, were exposed to fall hazards in that the employer failed to provide fall protection training to his employees prior to his employees working at heights greater than 10 feet 4 inches.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 13, 2025, employees engaged in roofing operations located at 11855 South 113th Ave, Papillion, Nebraska 68046, were being exposed to fall hazards in that the extension ladder positioned in front of the residential structure and was being used for access and egress to the roof did not extend at least 3 feet above the upper landing surface.  Instead, the employer had extended the ladder approximately 18 inches above the eave, denying the employees a safe hand hold as they transitioned on and off the ladder while standing near the edge of an approximately 10 foot 4 inch fall hazard.

29 CFR  1926.1053(b)(22):Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  On or about may 13, 2025, employees engaged in roofing operations located at 11855 South 113th Ave, Papillion, Nebraska 68046, were exposed to fall hazards in that an employee was observed climbing up an extension ladder while carrying a 40lb pound bundle of roofing paper that was positioned on top of his right shoulder.
