29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about May 13, 2025, at the above addressed jobsite, employees using hand pneumatic guns, were not wearing safety glasses, thereby exposing employees to hazards associated with eye injuries.  Cardinal Exteriors, Inc. dba Thomas Velasquez. was previously cited for a violation of this Occupational Safety and Health Standard 1926.102(a)(1) which was contained in OSHA inspection number 1733895, citation number 1 item number 1 and was affirmed as a final order on July 15, 2024, with respect to a workplace located at 806 N. Sutherland Ave., Palatine, IL.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about May 13, 2025, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18) feet eight (8) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about May 13, 2025, at the above addressed jobsite, employees performing residential roofing operations, seventeen (17) feet four (4) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  Cardinal Exteriors, Inc. dba Thomas Velasquez was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1783638, citation number 1 item number 1 and was affirmed as a final order on March 6, 2025, with respect to a workplace located at 45 Charles., Vernon Hills, IL.  Cardinal Exteriors, Inc. dba Thomas Velasquez was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1733895, citation number 1 item number 2 and was affirmed as a final order on July 15, 2024, with respect to a workplace located at 806 N. Sutherland Ave., Palatine, IL.

On or about May 13, 2025, at the above addressed jobsite,  employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
