29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about May 15, 2025, during a construction project at a residential home located at 6181 CROSSVIEW ROAD, SEVEN HILLS, OH 44134, the employer did not ensure employees working on the roof were protected from a fall of approximately 12 feet by utilizing a personal fall arrest system or any other form of fall protection system.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about May 15, 2025, during a construction roofing project on a residential building located at   6181 CROSSVIEW ROAD, SEVEN HILLS, OH 44134, the employer did not ensure employees working on a steep roof were protected from various fall heights of approximately 12 feet, by utilizing a guardrail systems with toe boards, safety net systems, or personal fall arrest systems. Employees were not provided training in fall protection by their employer.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  On or about May 15, 2025, during a residential construction roofing project located at 6181 CROSSVIEW ROAD, SEVEN HILLS, OH 44134, the employer did not ensure that the ladder rails on the portable ladder used by employee(s) to gain access to the roof were extended 3 feet above the landing surface.
