29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Jobsite, 150 Kriess Road, Butler, PA 16001:  On or about January 27, 2025, a Stevens International brand roll press identified as Press #17 (model number: MS-1000, serial number: 579106) was being operated without a nip bar to guard the in-running nip points created by spinning rollers on Printing Unit #1.  Each employee who operates the press was exposed to caught-by hazards.      b)  Jobsite, 150 Kriess Road, Butler, PA 16001:  On or about January 27, 2025, a Stevens International brand roll press identified as Press #17 (model number: MS-1000, serial number: 579106) was being operated without a nip bar to guard the in-running nip points created by spinning rollers on Printing Unit #2.  Each employee who operates the press was exposed to caught-by hazards.      c)  Jobsite, 150 Kriess Road, Butler, PA 16001:  On or about January 27, 2025, a Stevens International brand roll press identified as Press #17 (model number: MS-1000, serial number: 579106) was being operated without a nip bar to guard the in-running nip points created by spinning rollers on Printing Unit #3. Each employee who operates the press was exposed to caught-by hazards.      e)  Jobsite, 150 Kriess Road, Butler, PA 16001:  On or about May 19, 2025, a Stevens International brand roll press identified as Press #1 (model number: MS-1000, serial number: 94068) was being operated with the yellow guard in the raised (unguarded) position on Printing Unit #1.  Each employee who operates the press was exposed to caught-by hazards.  f)  Jobsite, 150 Kriess Road, Butler, PA 16001:  On or about May 19, 2025, a Stevens International brand roll press identified as Press #1 (model number: MS-1000, serial number: 94068) was being operated with the yellow guard in the raised (unguarded) position on Printing Unit #2.  Each employee who operates the press was exposed to caught-by hazards.  g)  Jobsite, 150 Kriess Road, Butler, PA 16001:  On or about May 19, 2025, a Stevens International brand roll press identified as Press #2 (model number: MS-1000, serial number: 94068) was being operated with the yellow guard in the raised (unguarded) position on Printing Unit #3. Each employee who operates the press was exposed to caught-by hazards.  i)  Jobsite, 150 Kriess Road, Butler, PA 16001:  On or about May 19, 2025, a Stevens International brand roll press identified as Press #1 (model number: MS-1000, serial number: 94068) was being operated with the yellow guard in the raised (unguarded) position on Printing Unit #5.  Each employee who operates the press was exposed to caught-by hazards.  j)  Jobsite, 150 Kriess Road, Butler, PA 16001:  On or about May 19, 2025, a Stevens International brand roll press identified as Press #2 (model number: MS-1000, serial number: 94068) was being operated with the yellow guard in the raised (unguarded) position on Printing Unit #6. 29 CFR  1904.39(a)(2):  The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  a) Jobsite, 150 Kriess Road, Butler, PA 16001:  On or about January 27, 2025, an employee was hospitalized after having three fingers on his right hand smashed between the rollers of a Stephens International roll press (model number: MS-1000, serial number: 579106).  While hospitalized, the employee underwent surgery that resulted in the amputation of three fingers on his right hand.  The employer did not report the work-related incident to OSHA until May 15, 2025.
