29 CFR  1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:  a) On or about May 16th, 2025 at 841 N Wheeling St Toledo, Ohio 43605 the employer Weber Contracting failed to ensure employees were protected from cave-in hazards while performing culvert replacement activities in a trench roughly 15-20 feet deep.
