29 CFR  1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question:  a) On or about May 16, 2025, at location 301 NW 82nd Ave, Plantation, FL, 33324, on the east  side of the medical establishment, the scaffold system was not properly inspected by the competent person that was not adequately trained to identify scaffolding deficiencies, exposing employees to a fall, struck by and/or crushed by hazard.
