29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.  a) Worksite: On or about May 19, 2025, employees operated a Groen Intek steamer reaching temperatures of over 200�F without readily available thermal hand protection. The unit was visibly damaged and secured with a red strap, exposing employees to burn hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   b) Worksite: On or about May 19, 2025, a Groen Intek steamer remained in use despite a broken door handle preventing the door from being properly closed. The unit was pressurized and operational, exposing employees to potential burn hazards from steam due to improper maintenance and lack of a secure closure.
