On or about May 19, 2025, at 985 Date Street, Merritt Island, FL: employees were exposed to 10 feet fall hazard while conducting roofing activities on a 4:12 pitched roof, in that, fall protection was not used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. On or about May 19, 2025, at 985 Date Street, Merritt Island, FL: employees were exposed to 10 feet fall hazard while conducting roofing activities on a 4:12 pitched roof, in that, the rails of the ladders did not extend at least three feet above the roof landing.
