29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a) Worksite, roof - On or about May 19, 2025, an employee was transitioning from a ladder to the roof of a church approximately 30 feet above the ground and did not attach to the Kwik Safety COBRA Self-Retracting Lifeline before exiting the ladder. The employee was struck by flying debris and fell to the ground below.
