29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) shall be installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform). a.) On or about May 21, 2025, the employer did not ensure that the space between adjacent scaffold planks was no more than 1 inch, in that a gap of approximately 1.5 inches was present between scaffold planks where employees worked. As a result, employees working on the scaffold were exposed to a tripping and fall hazard of approximately 13'.

29 CFR 1926.451(g)(4)(i):Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a.) On or about May 21, 2025, the employer failed to ensure that a complete guardrail system was installed along all open sides and ends of the scaffold platform before allowing employees to use it for work. As a result, employees working from the third level of the scaffold were exposed to fall hazards of approximately 13 feet.

All brace connections shall be secured. a.)
