29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: Main Warehouse (Dry and Frozen Goods): On or about May 21, 2025, and at times prior thereto, employees performance to operate a forklift were not conducted at least once every three years. Employees potentially exposed struck by hazards. In the alternative. 29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Main Warehouse (Dry and Frozen Goods): On or about May 21, 2025, and at times prior thereto, employees were permitted to operate four forklifts had not received training, nor been evaluated in the safe operation of the fork lifts. Employees potentially exposed struck by hazards.
