29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, caustic liquids, or chemical gases or vapors and at times prior thereto, workers were exposed to a struck by hazard while cutting siding with a saw, without the use and wear of protective eyewear: a. At a temporary worksite, located at the east side of 5535 Lot 42 Jacksonville FL: On or about May 27, 2025 the employer exposed employees to struck-by eye injury hazards from flying debris, in that protective eyewear use was not enforced by the employer while employees used metal hammers, and pneumatic nail guns for decking installation.

At a temporary worksite, located at the north side of 5535 Lot 42 Jacksonville FL: On or about May 27, 2025 the employer was exposing an employee to a 24 foot fall hazard, in that the employees were not tied off when installing the decking. a. At a temporary worksite, located at the east side of 5535 Lot 42 Jacksonville FL: On or about May 27, 2025 the employer was exposing an employee to a 12 foot fall hazard, in that the employees were not tied off when installing the decking.
