29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   62 Lawrence Plain Road, Hadley, MA: On or about May 27, 2025, employees were observed working on a residential structure engaged in roofing activities with unprotected sides and edges greater than 6 feet above the next lower level and were not protected by either guard rails with toeboards, safety net systems or personal fall arrest system exposing them to a fall from height hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  62 Lawrence Plain Road, Hadley, MA: On or about May 27, 2025, and at times prior, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed, and the signature of the person who conducted the training for the employees who were performing residential roofing and exposed to fall hazards of over 6 feet above a lower level.
