The wooden box, used as a scaffold, was not designed by a qualified person, exposing the employee to fall hazards.

29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use.  a) 12021 Rosedale Drive, Ellendale, DE 19941: On or about May 27, 2025, a JCB, model 509.42F, rough terrain forklift, was being used to support a wood box that was in use as a scaffold. The JCB was not designed for scaffold support by the manufacturer, exposing employees to fall, crush, and struck-by hazards.
