29 CFR  1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:   Site- Workers under the control of the employer working near second floor window openings were not protected from a fall of approximately 9 feet where the bottom sills of the window openings were 22 inches from the floor and no top rail was installed.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:  Site- Workers under the control of the employer were not protected from trip hazards where the  stairways inside each unit, from the first floor to the second floor, did not have handrails.
