29 CFR 1926.20(b)(1): The employer failed to initiate and maintain such programs as may be necessary to comply with this part.  On or about June 3, 2025, the employer did not have a program in place to identify hazards found in the job site, 2395 Bowman Street Rd in Mansfield, Ohio 44903. This program assists employees and the employer identifying and avoiding potential hazards such as, but not limited to, falls from heights and the use of personal protective equipment required for assigned job tasks.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about June 3, 2025, the employer working at 2395 Bowman Street Rd in Mansfield, Ohio 44903 did not ensure their employees were protected from fall hazards when removing roofing material from a residential building. Employees operating in this condition were exposed to falls of approximately 7 ft. to the level below.
