On or about June 3, 2025, the employer did not have a program in place to identify hazards found in the job site, 302 Maplewood St in Willard, Ohio 44890. This program assists employees and the employer identifying and avoiding potential hazards such as, but not limited to, fall and the use of personal protective equipment required for assigned job tasks.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about June 3, 2025, the employer working at 302 Maplewood St in Willard, Ohio 44890 did not ensure their employees were protected from fall hazards using appropriate fall protection measures when removing roofing material from a residential structure. Employees operating in this condition were exposed to falls of approximately 10 ft. to the level below.

29 CFR 1926.503(a)(1):The employer failed to provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about June 3, 2025, the employer working at the residential site 302 Maplewood St in Willard, Ohio 44890 did not ensure their employees had been adequatly trained on the appropriate use of fall protection systems including but not limited to, personal fall arrest systems or alternative fall protection measures. Employees were removing shingles and preparing to secure new shingles to the roof of a multi-story residential building without the benefit of fall protection training, exposing employees to falls hazards of over 10 ft..
