29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment shall be guarded if such parts are exposed to contact by employees or otherwise create a hazard. Guarding shall meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), Safety Code for Mechanical Power-Transmission Apparatus. (a) On or about June 3, 2025, employee(s) used an extension ladder hoist to convey materials up to the roof. The hoist did not have the power transmission pulley and belt guarded, exposing employees to a caught-in hazard and amputation hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about June 3, 2025, employees conducting roofing work on a 6:12 pitch roof, on the second story of a residential home were not using fall protection. This exposed employees to a fall hazard of approximately 18 feet from roof to the ground below. (b) On or about June 3, 2025, employee conducting tear off work on the first story of a residential roof with a 4:12 pitch, was not tied off. This exposed employee to a fall hazard of approximately 8 feet from the roof to the ground below.
