29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) Worksite was located at 180 NJ 35, Eatontown, NJ 07724:  A worker was exposed to fall hazards of approximately twenty feet to gravel while working to install a commercial HVAC unit on the roof of a new retail store under construction. The roof had multiple skylights that were open, unguarded holes. The worker and the crew he was working with did not utilize fall protection while performing this task.  Violation occurred on or about June 2, 2025.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
