29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  a) At the jobsite located at 2010 Celsius Way, Davenport, Florida - On or about June 4, 2025, the employer did not provide frequent and regular inspections of job sites, materials, and equipment by a competent person to identify and correct safety and health hazards.  Employees were exposed to crush/caught-between and impalement hazards while performing concrete flatwork.

The employer failed to make provisions prior to the commencement of work on the project exposing employees to crush and caught-between hazards.

29 CFR  1926.701(b):  All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:  a) At the jobsite located at 2010 Celsius Way, Davenport, Florida - On or about June 4, 2025, the employer exposed employees to impalement hazards , in that, multiple pieces of rebar which extended up to 6 inches above the top of concrete sidewalk forms were not capped.
