29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: At a temporary job site located at 1506 Lilac Lane, Lawrence, Kansas , on or about June 4, 2025, employees were exposed to struck-by and engulfment hazards in that there was no safe means of egress in an approximately 12 feet 2 inch trench. The employees was using an extended aluminum ladder that was broken and used incorrectly.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At a temporary job site located at 1506 Lilac Lane, Lawrence, Kansas, on on or about June 4, 2025, employee was exposed to struck-by and engulfment hazards. The employee was working in a trench measured at approximately 12 feet, 2 inches deep without adequate shoring, sloping or benching as a means of protection against trench collapse. The trench had caved in on one end near Takeuchi TB290 back hoe and on left, front side near wall brace.
