29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about: June 4, 2025 Location: 242 N Broadway Sleepy Hollow, NY 10591 Eastern building behind main church  a) Three workers were observed not utilizing personal fall arrest systems (PFAS) while performing work on church roof at heights of approximately 40-45 feet. Workers exposed to fall hazard due to lack of fall protection.
