29 CFR  1910.215(a)(4):  Work rests on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a)SFK Leasing, Inc. dba Custom Truck Leasing, Inc. at 4400 North Cliff Avenue, Sioux Falls, SD 57104:  On or about and at times prior to June 5, 2025, the employer did not ensure that the work rest on the Dewalt Bench Grinder (model: DW758, s/n 2007_13-YL1722) located in the east maintenance bay, was adjusted with a maximum opening of one eighth inch.  This condition exposed employees to struck-by and caught-in hazards.
