29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about 06/06/2025, at 314 Smelkoff Rd., Syracuse NY 13209 : Employees were using an extension ladder to access the house roof and the ladder was not extended 3 feet above the edge of the roof exposing employees to a fall hazard of 122 inches.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about 06/06/2025, at 314 Smelkoff Rd., Syracuse NY 13209 : Employees were installing siding on a house while working from a section of the house roof with an estimated 4:12 pitch roof without fall protection of any kind, exposing employees to a fall hazard that measured 122 inches above the ground.   Mike Home Improvements Corp. was previously cited for an equivalent violation of the safety and health standard 1926.501(b)(13), which was contained in OSHA inspection 1678442, citation 1, item number 1(a) and was affirmed as final order on 3/6/2025, with respect to a workplace located at Bayshore Apartments, 9468 Chalkstone Course, Brewerton, NY 13029.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about 06/06/2025, at 314 Smelkoff Rd., Syracuse NY 13209 : Employees  installing siding on a house, while working from a section of the house roof without fall protection of any kind, were exposed to a fall hazard and had not been trained in the recognition of fall hazards and fall hazard protection.   Mike Home Improvements Corp. was previously cited for an equivalent violation of the safety and health standard 1926.503(a)(2), which was contained in OSHA inspection 1678442, citation 1, item number 1(b) and was affirmed as final order on 3/6/2025, with respect to a workplace located at Bayshore Apartments, 9468 Chalkstone Course, Brewerton, NY 13029.
