As a result, employees were exposed to fall hazards greater than 20 feet while performing residential construction.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used. a) Located at Lot 334-5433 Morning Glory St, South Bloomfield, OH 43103: On or about June 6, 2025, each employee on the jobsite was not protected from electrical shock hazards in that the employer did not prevent worn or frayed electric cords or cable from use. The orange and black extension cord, being used to provide temporary power to a microwave, had damaged insulation at the female plug end.

29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a) Located at Lot 334-5433 Morning Glory St, South Bloomfield, OH 43103: On or about June 6, 2025, the employer did not ensure that employees framing were protected from falling by a fall protection system. As a result, employees were exposed to a fall hazard of greater than 21 feet while performing residential construction. b) Located at Lot 334-5433 Morning Glory St, South Bloomfield, OH 43103: On or about June 6, 2025, the employer did not ensure that employees framing the interior of the home were protected by an adequate guardrail system in that the second-floor stairwell did not have a guardrail installed. As a result, employees were exposed to a fall hazard greater than 9 feet.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) Located at Lot 334-5433 Morning Glory St, South Bloomfield, OH 43103: On or about June 6, 2025, the employer failed to ensure that each employee exposed to a fall hazard was trained on the hazards of falling and the procedures to minimize the hazards in the work area. As a result, employees were exposed to fall hazards greater than 20 feet while performing residential construction.
