29 CFR 1910.119(e)(3)(i): The Process Hazard Analysis did not address the hazards of the process: a) In the ECU, on or about June 4, 2025 - The Process Hazard Analysis (PHA) did not cover the hazards of the process during decoking operations and the subsequent startup to return to normal cracking operations. Failure to address the hazards of the process allowed MOV-511 and MOV-513 to be inadvertently opened at the same time allowing cracked gas to enter the Furnace #5 firebox from the cracked gas header resulting in an explosion and fire.

29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not address human factors: a) In the ECU, on or about June 4, 2025 - The employer's Process Hazard Analysis (PHA) did not consider or adequately address human factors for non-routine operations such as what happens if a critical step is performed wrong when MOV-511 and MOV-513 are returned to normal operation after being isolated for furnace maintenance. Failure to address human factors in the PHA allowed MOV-511 and MOV-513 to be inadvertently opened at the same time allowing cracked gas to enter the Furnace #5 firebox from the cracked gas header resulting in an explosion and fire.
