29 CFR  1926.403(b)(1):  The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) Employee(s) using a 100 foot flexible cord that had multiple areas of damage was being used to power / charge portable tools located at 9026 Shady Bend, Lenexa, Kansas were exposed to electrical hazards due to the damage areas exposing the inner energized conductors.  (b) Employee(s) using a multiple outlet extension cord that had several areas of damage was being used to power / charge portable tools located at 9026 Shady Bend, Lenexa, Kansas were exposed to electrical hazards due to the damage areas exposing the inner energized conductors.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  Employee(s) framing the master bedroom ceiling for a residential building approximately 10 feet - 1/2 inches from ground level located at 9026 Shady Bend, Lenexa, Kansas were exposed to a fall hazard.  The framing worker(s) was not using a fall protection system to prevent fall hazards.
