Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about June 5, 2025, employees were exposed to fall hazards of approximately 10 feet on a construction site in Springfield, Illinois, and adequate fall protection was not being used while working on a low-slope commercial building.
