On or about June 11, 2025, at the above addressed jobsite, anchorages being used for attachment of personal fall arrest equipment were not installed so that they would be capable of supporting at least 5,000 pounds per employee attached, in that, two employees were attached to the same anchorage point. Having more than one employee attached to anchorage point, invalidates the manufactures safety factor of at least two thereby exposing employees to fall hazards.
