29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  (a): Interior - Third Floor - Employees working on the third floor were exposed to fall hazards through an unprotected hole in the floor. An employee sustained injuries when he fell approximately 10 feet through the hole, landing on second floor below, on or about December 26, 2024.  There is no abatement certification or documentation required for this item.
