(c) 5104 Old US 22, Hamburg, PA; In the big and small warehouse - On or about June 12, 2025, the anchor plates were not bolted to the floor at any of the racking systems. Employees were exposed to crushed by and struck by hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
