29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Residential Site: On or about 6/11/2025, and at times prior, the employer exposed employees to fall hazards, in that a complete fall protection system was not provided to employee(s) while they were performing siding activities on a 10:12 pitch roof at heights more than 12 feet from ground level.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Residential Site: On or about 6/11/25, and at times prior, the employer exposed employees to fall hazards, in that employees were using a portable ladder to access the roof that was only extended approximately two feet above the landing surface while performing siding activities.

29 CFR  1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:  a) Residential Site: On or about 6/11/25, and at times prior, the employer exposed employees to fall hazards, in that employees were using  a Werner portable ladder that was placed on uneven ground while employees were using the ladder to access the scaffold to perform siding activity .
