At the jobsite located at 6611 Dearborn Drive, Mission, Kansas, employees using a portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 8 feet from the roof to the ground. The side rails of the portable extension ladder were not secured and did not extend at least 3 feet above the landing surface of the roof.
