29 CFR 1926.416(a)(1): The employer did not prevent employee(s) from working in proximity to an electric power circuit that the employee could contact during the course of work and the employee(s) were not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means:  a) On or about 06/13/2025 at the site located at 6879 Buckley Road, Syracuse, NY, 13210: The employer did not protect employee(s) from electrical shock when roofers were using multiple tools, including but not limited to a metal scrapers in close proximity to incoming 240-Volt electrical lines.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) On or about 06/13/2025 at the site located at 6879 Buckley Road, Syracuse, NY, 13210: Employees were exposed to fall hazards when working on a 4:12 and 2:12 roof without the use of fall protection. Employees were exposed to fall hazards of approximately 9 feet and 15 feet.
