29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a.  At a temporary worksite, located at the east side of 1205 Tribute Circle Lot 38 Jacksonville FL:  On or about June 13, 2025, the employer exposed employees to struck-by eye injury hazards from flying debris, in that protective eyewear use was not enforced by the employer while employees used electrical saws, metal hammers, and pneumatic nail guns for trusses installation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a.  At a temporary worksite, located at the east side of 1205 Tribute Circle Lot 38 Jacksonville FL:  On or about June 13, 2025, the employer exposed employees engaged in framing activities to fall hazards up to 23 feet to the ground below in that the employees were allowed to work at heights without an adequate personal fall arrest system.
