At a temporary worksite, located at the east side of 1205 Tribute Circle Lot 38 Jacksonville FL: On or about June 13, 2025, the employer exposed employees to struck-by eye injury hazards from flying debris, in that protective eyewear use was not enforced by the employer while employees used electrical saws, metal hammers, and pneumatic nail guns for trusses installation.

At a temporary worksite, located at the east side of 1205 Tribute Circle Lot 38 Jacksonville FL: On or about June 13, 2025, the employer exposed employees engaged in framing activities to fall hazards up to 23 feet to the ground below in that the employees were allowed to work at heights without an adequate personal fall arrest system.
