29 CFR  1904.41(a)(1)(i):The establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, and the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by the specified date:  a) On or about March 2, 2025, the employer failed to electronically submit information from their OSHA Form 300A, Summary of Work-Related Injuries and Illnesses or equivalent form, for calendar year 2024 on or before March 2, 2025. The establishment employed 60 employees during calendar year 2024 and was classified in the NAICS code 238910.

29 CFR  1926.1053(b)(16):Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  A) Excavation located at 9th Ave and N. Madison St.: On or about June 12, 2025, the employer exposed employees to a fall hazard in that the fiberglass ladder used to enter and exit the excavation had a damaged side rail and it was not tagged or marked as being defective.
