Pitt Ohio Express, Inc. On or about 6/17/25 and times prior thereto: the brake pedal on the Uni Carriers F9569 Forklift (Model- MCP1F2A20LV, Serial No.- CP1F2-9W35026) was missing the rubber grip/anti-slip texture on the face of the pedal.  B.) Pitt Ohio Express, Inc. On or about 6/17/25 and times prior thereto: the clutch pedal on the Uni Carrier F9552 Forklift (Model-MCP1F2A20LV, Serial No.-CP1F2-9W34445) was missing the rubber grip/anti-slip texture on the face of the pedal.  NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED

On or about 6/17/25 and times prior thereto: safety labels attached to the Uni Carriers F9569 Forklift (Model- MCP1F2A20LV, Serial No.- CP1F2-9W35026) was not identifiable/legible due to message being worn off from wear.     B.) Pitt Ohio Express, Inc. On or about 6/17/25 and times prior thereto: safety labels attached to the Uni Carriers F9552 Forklift (Model-MCP1F2A20LV, Serial No.-CP1F2-9W34445 was not identifiable/legible due to message being worn off from wear.     NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
