29 CFR  1910.212(a)(1):Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks:  On or about June 17, 2025, at a  workplace located at 15750 South Keeler Terrace in Olathe Kansas, employees were exposed to caught in and crushing hazards in that the employer did not ensure the rotating parts were guarded allowing an employee's hand to come onto contact resulting in an amputation. Employees were exposed in the following instances: a) Rotating part on the CNC Mill - Sharp SN: 21128001 was not provided with chuck guards b) Rotating part on the Shop fox drill was not provided with chuck guard
