29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. a. Located at 6024 Hoover Road, Grove City, Ohio, 43123, on or about June 17, 2025, while performing roofing work, employees were exposed to a fall hazard of approximately 9 feet 10 inches. An extension ladder was used to access the roof; however, the employer did not ensure that the ladder extended at least 3 feet above the upper landing surface, as required. This failure increased the risk of falls during access to and from the roof by employees.

29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. Located at 6024 Hoover Road, Grove City, Ohio, 43123, on or about June 17, 2025, the employer failed to ensure that employees engaged in framing a one-story, single-family home were protected by an appropriate fall protection system. As a result, employees were exposed to fall hazards of up to approximately 9 feet 10 inch to the ground below.
