29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  (a) On or about June 17, 2025, at the jobsite located at 2562 N 72nd Street Wauwatosa WI, the employer did not ensure that employees installing underlayment on a residential property with a 12:12 pitch were protected from falling to the lower level, exposing employees to a fall hazard of approximately 18 8/10 feet to the ground below.

29 CFR  1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  (a) On or about June 17, 2025, at a jobsite located at 2562 N 72nd Street Wauwatosa WI, the employer did not ensure the anchorage points were installed correctly exposing employees to an 18 foot fall hazard to the ground below.  The fall protection anchors that employees were using were not installed properly, in-that only one of the two sides of the roof anchor point was secured to the roof.

(a) On or about June 17, 2025, the employer had not retrained and verified employees understood the proper methods of installing and utilizing fall protection equipment/systems.
