(a) On or about June 17, 2025, the employer had not ensured that employees performing roofing work had been trained to recognize and minimize the fall hazards associated with their work, exposing employees to a fall hazard.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about June 17, 2025, the employer did not ensure that employees performing tear off of a two-story residential home roof were protected from falling to the ground below. This Exposed the employees to a fall hazard of approximately 21 feet.  Chagos Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1815503, citation number 1, item number 1 and was affirmed as a final order on June 10, 2025, with respect to a workplace located at 1146 W. Violet Drive, Oak Creek, WI 53154.  Chagos Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1815503, citation number 1, item number 2 and was affirmed as a final order on April 28, 2023, with respect to a workplace located at 810 W. South Street, Oconomowoc, WI 53066.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) On or about June 17, 2025, the employer did not ensure the extension ladder used by employees to access and egress the residential roof was extended at least three feet above the upper landing, exposing employees to a fall hazard of approximately 21 feet to the ground below.  Chagos Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1815503, citation number 1, item number 2 and was affirmed as a final order on June 10, 2025, with respect to a workplace located at 1146 W. Violet Drive, Oak Creek, WI 53154.
