29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems. a) On or about June 20, 2025, the employer failed to ensure that employees installing roofing materials on a 4.8/12 pitch commercial roof were utilizing a personal fall arrest system, exposing employees to fall heights greater than 8 feet.

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) On or about June 20, 2025, the employer failed to ensure that each employee exposed to a fall hazard were trained on the hazards of falling and the procedures to minimize the hazards in the work area. Employees were exposed to fall hazards greater than 8 feet while performing commercial roofing work.
