29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Residential roofs, various locations;  On or about June 17, 2025, one or more methods of fall protection was not in place while employees performed roofing activities at heights ranging between approximately 8-feet, 5-inches to 20-feet, 4-inches above ground level.

(b) Brookstone Townhome 1952, south side;  On or about June 17, 2025, an extension ladder placed at ground level and used for accessing the roof did not have the side rails extended at least 3 feet above the landing surface.
