29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about 06/24/2025, the employer failed to ensure that the C73 Baffle Tester was properly guarded. The back side and top portion of the machine were not equipped with guards to prevent employees from reaching into the point of operation and contacting the ingoing nip point and rotating parts while the machine was operating. As a result, employees were exposed to amputation hazards, including the incident on 06/13/2025 in which an employee sustained a thumb amputation when reaching into the point of operation.
