29 CFR  1910.23(c)(11): Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface;   On or about January 13, 2025, at a worksite located at 4320 Brauer Road, Sturgeon Bay, Wisconsin; an employee utilizing a portable ladder that did not extend three feet above the roof eave exposing the employee to a fall hazard over seven feet.

29 CFR  1910.28(b)(1)(i): Unprotected sides and edges. Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: (A) Guardrail systems; (B) Safety net systems; or (C) Personal fall protections systems, such as personal fall arrest, travel restraint, or positioning systems.  On or about January 13, 2025, at a workplace located at 4320 Brauer Road, Sturgeon Bay, Wisconsin; employees were exposed to fall hazards of over six feet while conducting internet servicing when they were not protected by guardrail systems, safety net systems, or personal fall protections systems, such as personal fall arrest, travel restraint, or positioning systems.

29 CFR  1910.178(l)(2)(iii): All operator training and evaluation shall be conducted by persons who have the knowledge, training, and experience to train powered industrial truck operators and evaluate their competence:   On or about June 24, 2025, at a workplace located at 300 Industrial Drive, Random Lake, Wisconsin; the employer did not ensure each employee performing forklift operator evaluations was permitted to operate a lift and had the knowledge, training, and experience to adequately evaluate operators' competency.
